Karen takes the form of a documentary; viewers shadow a private investigator as she follows a client's sister. The private investigator, Caroline Chase, narrates as she works to prove that the sister in question, Ashley Graves, is what modern society classifies as a "Karen." A "Karen" usually describes a privileged white woman with a very distinct hairstyle (short and blonde) that is always angry, entitled, and interfering in situations that are none of her business. Caroline is hired by Rachel, Ashley's twin sister. Early on, Rachel provides her guidance to the search, but then disappears until the end as Caroline delves deeper into Ashley's lifestyle, witnessing her hairstyle change and angry conversations. Caroline films Ashley's ridiculous encounters with other people, supplemented by eyewitness accounts and telephone call recordings. As Caroline's investigation draws to a close, she catches Ashley mailing some mysterious essential oils to her sister Rachel. When she follows Ashley to a restaurant, she discovers her to be having a meal with Rachel, who has now transformed into a "Karen" as well. They notice Caroline filming them, chase after her, and threaten to sue her. This documentary comes to an abrupt close as Caroline is forced to conclude it due to the lawsuit.
