In the year 2056 - the not so distant future - an epidemic of organ failures devastates the planet. Out of the tragedy, a savior emerges: GeneCo, a biotech company that offers organ transplants, for a price. Those who miss their payments are scheduled for repossession and hunted by villainous Repo Men. In a world where surgery addicts are hooked on painkilling drugs and murder is sanctioned by law, a sheltered young girl searches for the cure to her own rare disease as well as information about her family's mysterious history. After being sucked into the haunting world of GeneCo, she is unable to turn back, as all of her questions will be answered at the wildly anticipated spectacular event: The Genetic Opera.

A character received a botched face transplant, which is seen falling off at the top shortly after surgery, and she is seen pressing it back up like a mask or sticker. A little later in the movie her face falls completely off. Underneath it's just the actress's face painted with blood, and she doesn't appear to be in pain, so it's not horribly gorey.

A woman is chased by the Repo man in the beginning. When he finds her, he slits her throat and we see lots of blood pouring out of the wound, and he pulls her heart out

Pictures of Pavi stealing the face of a woman are seen.

A doctor is shot in the head and killed; there is blood spray.

We see a cave-like area with dead bodies. Some of them are bloodied, some of them are not. GraveRobber is shown sticking a needle in dead bodies and extracting Zydrate.

Shiloh falls unconscious several times.

A man is shown tied to a chair in one scene. Nathan cuts out his organs (brief glimpses of him stabbing the man can be seen). And after the song it shows his body again, gutted and half decapitated.

Nathan is shown trying to save Marni during her birth, who loses a lot of blood and dies. He cuts her stomach open and saves Baby Shilo, covered in goo and blood.

Luigi is bumped into by a Gentern carrying a brain, leaving a huge blood stain on his shirt. Frustrated, he stabs her in the stomach, and we see her fall into the desk and the desk falls on her. She is seen slowly dying in a pool of her own blood while everyone else in the scene ignores her.

Luigi picks up a brain and tosses it to a Gentern, telling her to "mark it up".

A man is shown tied up and ready for repossession. Nathan stabs him in the stomach and pulls out his intestines and heart. The man is shown leaking blood all over the floor and Nathan's suit, which Nathan washes the whole place by spraying it down with a hose later on.

Luigi stabs a person randomly at the opera preparation place, no blood is shown.

A man is hung by his ankle and knocked out. The Repo Man kills him by breaking his neck, slices his back open, and repossesses his spine, we see a graphic shot of the spine covered in blood. The Repo Man later seen handing it to Rotti during "Who Ordered Pizza?".

Nathan is shown slashing a victims' chest in one scene, before the scene ends, we see a graphic shot of the victim's stomach sliced open with the intestines falling out.

Amber is shown getting surgery during "Zydrate Anatomy". We don't see much but the scalpel slicing into her skin and some bleeding.

It is revealed Amber's face was badly disfigured during a surgery; we see the resulting face.

There are people with surgery stitches and scars everywhere in this film.

Nathan slits a few of GeneCo's guards' throats. He is later seen stabbing a few more.

Amber is shown trying to keep her face on before the show. During "Blame Not My Cheeks", she struggles to keep it on, and then she does a faulty spin and her face falls off. This is done for comic effect.
